Thank you for all the suggestions. Here are some further clues. Please 
help me troubleshoot this:

1. Checked alignment. Maybe 1-16th to 1-8th off. Don't think this is the 
problem because I drove 5000 miles with the same alignment before this 
started happening.

2. Noticed some intermittent underhood whine noise around the time this 
started happening.

3. Water pump and fan turn OK by hand (no fan clutch). 

4. Belt does not break at low speeds (up to 55 mph). It loosens up, but 
is not real hot.  

Could the water pump only malfunction at high speeds (resist turning) 
and cause a strain on the belt?
