For the Disc Brake Conversion, you can look in wrecking yards and locate some early a-bodies that have the DISC Brake setup with the 5 on 4 bolt pattern (Small Bolt Pattern). Or Convert to the larger 5 on 4 -1/2 bolt pattern.

I know I picked up a setup from a '65 Formula S for mine for about $300 from a wrecking yard in Colorado. if you find an early A-body owner, you will want to pull the Spindles, Calipers, Backing Plate, Brake Lines (So you can copy them from MC to the Calipers) and the proportioning valve. I grabbed the lower control arms also, just so I would have spares. I still have yet to rebuild the calipers and install mine. Maybe this fall.

These calipers are the 4-piston and can be a pain to rebuild, but it is do-able

For the other option for 5 on 4 1/2, I think there is an article on the 62 to 65 site.

    I am currently looking for parts and info for my 1964 Dart GT.  Mainly disc. brake conversions for the front. minor trim pieces, Window moldings, ect....  My car is fairly in good shape.  Only a phew things to work on.  Mainly the slant 6 rebuild.  I am forward deployed in Naval forces Japan and I cant wait to get home and work on my Dart.  Sick and tired of turning wrenches on rice rockets and F/A18 Hornets.  I am trying to make sure I have parts at home ready to go.  Please E-mail me with any helpful hints.
