This came from information that Rob Main posted years ago but I just found today.

Bulletin J14 dated February 29, 1956. Apparently there was more information on the back of this page. I don't have that. Rob didn't seem to have it either.

It says in the first paragraph: Distribution (shipping cost), Excise (tax) and Handling Charge (fees).
