All: I agree totally with Joe Savard about the quality of the
fasteners (bolts and washers) you got. From my past work experience, I
learned that the US is flooded with counterfeit fasteners. What makes them
counterfeit is that they are made in a foreign country out of scrap metal of
unknown quality and marked as grade 5 or 8 fasteners.
How do you get good fasteners? I
don't know for sure, but try to determine if your supplier will stand behind his
products, or buy from reputable supplier.

can think of is that you got some of the wrong grade bolts/washers. When
I was a Department of Defense inspector, we had a series of cases where
bolts were wrongly marked, and failed. They were too soft.
