This is just a guess, and probably an obvious one at that, but often the 
obvious is overlooked ...

Have you changed the ballast resistor recently?  Perhaps the wrong 
resistor was installed ... or is faulty ... and it is not appropriately 
limiting voltage (it's purpose)?
