There is a BIG difference between re-built and remanufactered. Re-built is a replace what is broken only and put it in the box. Re-man has a criteria of replacement parts that are replaced wether bad or not. It's well worth the price difference in labor and aggravation of replacing and returning defective parts to the parts store. I spent about 20 years in the parts business and have seen about everything you could imagine. When using headers I try to create a heat shield of some sort if possible and I never tell the guy at the counter I have headers.

Yeah, rebuilts are dependent on the state of mind of the person rebuilding it that day, as well as the policy of the
company doing the rebuilding: often it is "replace only the minimum" -- not all.

That's an extreme example of rebuilt parts failure. Bummer indeed. And the ease of getting to the starter is a factor: when headers are involved you don't want to ever mess with R&R the starter again. Not so much an issue with cars with the original exhaust manifolds.
