If the balancer has slipped that would cause a permanent and unchanging shift in the timing.  A fluctuating idle is generally a sign of a vacuum leak or carburetor trouble or both, but do not kill yourself looking for it until you are certain your ignition system is up to par.  Less common but with the same symptom, if your timing chain has not been replaced in the last 50,000 miles it is suspect and may have stretched.  Fix the electrical stuff first, because that's what makes the spark happen, then look for reasons why it does not happen at the time it is supposed to.
